The member forms library holds seventeen fillable PDFs covering the paperwork side of the work. These are the documents you hand to counsel, file with a deliverable, or keep for your own record.

How to use them
Every form is a fillable PDF. Open it in any PDF reader, type into the fields, and save or print. You do not need Adobe Acrobat Pro. Preview on a Mac and Edge on Windows both fill these correctly.
Fields will not fill
If the form opens but the fields are inert, you are almost certainly viewing it in a browser's built-in preview rather than a real PDF reader. Download the file to your machine and open it locally. This is the cause roughly every time.
Can I put my own logo on them
Yes. These are working documents, and members routinely rebrand them for their own practice. What you should not do is present an AGCV form as certifying something it does not, or imply Guild endorsement of a specific job.
A form you need is not there
Tell us. Email info@agcv.com with what the form has to do and what jurisdiction it is for. Several of the seventeen exist because a member asked for them, and the library is meant to keep growing that way.
State-specific versions
The forms are built against federal practice and the common state patterns. If your jurisdiction requires specific language the form does not carry, do not assume the form is sufficient. Check the local rule and the judge's standing order, and ask in the Legal Video Forum if you want a second read from someone who practices there.
